If you see this in an app-killer application please do a refresh and check again. The large widget receives a tick every minute, while the small one receives it every second. If the widget is not visible (on a different home screen, covered by an open app), then its graphics are not updated (this however depends on your home screen implementation). When the device is sleeping the ticks are not delivered at all.
On my HTC Hero it does not eat more than 3% cpu when updating the widget, and that is a very weak device. Your mileage may vary, however.
